PURPOSE OF POSITION: As a part of the Clinical Concierge Team, the Coordinator will collaborate with divisional clinical staff and families to develop an interdisciplinary itinerary of clinical appointments for patients, primarily those living outside CCHMC's primary service area.

MAJOR DUTIES AND RESPONSIBILITIES:

1. Receive referrals from divisions, families and referring physicians to identify all clinical resources required to care for the child.

2. Complete intake questionnaire with the family to attain patient clinical history, current needs, and non-clinical preferences.

3. Utilizing the Tempus and Epic scheduling systems, create clinical itinerary for multiple subspecialty appointments that address the families clinical needs in the most efficient manner.

4. Collaborate with divisional representatives to make arrangements on an urgent basis or if appointments are not available.

5. Collaborate with Guest Services Associates to make logistical arrangements, such as transportation and accommodations

6. Communicate care plan to clinicians, families and referring physicians

7. Serve as a conduit to other hospital resources, including Social Services, Family Resource Center, International Health, Interpreter Services, Financial Counseling

8. Monitors the arrival of out-of-town guests to ensure patients and families reach their internal destination with care providers.

9. Participate in improving organizational performance by recommending areas or approaches for improvement activities, performing new procedures, collecting data and providing input to department discussions.

10. Ensure that work areas are organized and present a safe, accessible, effective and efficient environment for employees, patients and families.

11. Attend required Cincinnati Children's training sessions, including fire safety, electrical safety and OSHA. Comply with Infection Control policies and procedures.

12. Comply with policies and procedures protecting patient rights for informed consent and the handling of confidential information as defined by Cincinnati Children's mission and applicable laws and regulations. Review timeliness, accuracy, availability and security of information.

13. Participate in improving organizational performance through recommending areas or approaches for improvement activities, performing new procedures, collecting data and providing input to department discussions.

14. Participate in establishing job requirements and goals; perform duties at the desired level of competency.

15. Ensure the continuous and timely availability of nursing services to patients.

16. Actively participates as a front line example of extraordinary customer service. Participates in customer service initiatives that are appropriate.

17. Understands, adheres to and models Core Standards as defined organizationally and specifically within the department/unit.

18. Perform other duties as assigned.

REQUIRED SKILLS: Possess expert knowledge of the growth and development of pediatric patients and in the evaluation of deviations from the norm. Exemplary verbal, written and interpersonal communication skills. Capable of relating to diverse age and demographic backgrounds. Demonstrate diplomacy, tact and professional demeanor. Ability to problem-solve, work independently and collaborate in small teams. Exceptional organizational skills required in order to coordinate/maintain multiple tasks. Demonstrated proficiency in computer and word processing, spreadsheets, and database management

EDUCATION/WORK EXPERIENCE: Registered Nurse and current license to practice nursing in the State of Ohio required. Extensive clinical experience collaborating with multiple CCHMC divisions to deliver multidisciplinary care. Experience in Care Management preferred.
